ENROLLMENT AGREEMENT

Before we begin, please read this agreement, as it covers some of the issues of law governing our relationship.

1. Energy In Symmetry: provides a holistic approach to the mind, body, and spirit. The person you will be working with is a Board Certified Master Practitioner and is not a licensed Medical Doctor, psychiatrist, psychologist, M.F.C.C., or M.S.W. You acknowledge and agree that this session is not a substitute for medical counseling and should be used along side of any treatment and medications you are presently prescribed.

2. You Consent: to a Reiki Session treatment and understand that Reiki is an energy healing practice to help reduce stress and relax the mind, body, and spirit. You acknowledge that Reiki is a safe and gentle method that has no known side effects and cannot harm the body in any way.

3. You Understand: If you experience an emergency, a worsening of any health condition, or new ailment arises that it was not cause by Reiki, and you should seek a medical professional immediately.

4. After the Session: Your practitioner cares about you. The next day you will receive a follow up call to verify how you felt, feel and any energy shifts to note on.

It is very important that you understand the bodies "healing crisis". This is a huge shift in energy that can leave you feeling, tired, nauseous, or even irritable. Please read the reiki aftercare brochure on the website found on the Reiki page. Many times we are under stress for too long without release, and the energy shift through the body can be significant.

5. During your session we will use the following techniques:

6. Confidentiality: All client information is kept strictly confidential and is for internal use only.

7. Responsibility: It is the responsibility of the client to take action and confirm changes that produce the best results. It is the clients responsibility to listen to suggestions (found in the aftercare brochure) and communicate results. Energy In Symmetry has a liability that is limited to the amount paid for the initial session only.
